113. Reconstruction and Mechanics of the Middle Ear Lab, Massachusetts Eye & Ear, Boston, MA

The Reconstruction and Mechanics of the Middle Ear lab under Aaron Remenschneider, MD, MPH at Massachusetts Eye and Ear in downtown Boston studies a variety of pathologies and novel engineering solutions to problems faced in the middle ear, particularly with respect to the tympanic membrane (eardrum). The tympanic membrane is a thin tissue that captures sound waves from the environment and transmits them to the ossicles (bones) of the middle ear. Current projects in the lab include 3D printing of novel biodegradable grafts for tympanic membrane repair, investigating tympanic membrane remodeling in chinchilla models and human histopathological samples, and studying the acoustics of the middle ear following various reconstruction procedures. We also use human temporal bone specimens to improve our understanding of the mechanics of the middle ear and the impact of novel devices on hearing.

Age- and noise-related hearing loss present a major global health challenge worldwide. Specifically, over 5% of the population has disabling hearing loss, affecting approximately 466 million people worldwide each year. Current hearing aids seek to increase the sound pressure level that reaches the tympanic membrane, or eardrum, via amplification of sound waves in an electronic microphone-to-speaker system. Unfortunately, these hearing aids present challenges with maintenance, battery life, and environmental usage restrictions. Thus, patient adherence can be low for these devices. Our team seeks to create a passive hearing aid device utilizing novel materials and 3D printing techniques to augment the structure of the tympanic membrane. We hypothesize that by augmenting the circular/radial structure of the tympanic membrane, which has been shown to be important for enabling sound conduction at both low and high frequencies, we may be able to enhance the sound-induced motion of the tympanic membrane to sound waves. In this way, we may be able to develop a fully implantable device that enhances the motion of the ossicles and therefore cochlear fluid to improve hearing.
